This policy describes information handled by cohenmehreye.com. Medical records and protected health information are governed separately by our Notice of Privacy Practices under HIPAA, available at the office on request.

02What we collect

Information you choose to send, and basic site data.

Appointment & intake forms

Booking requests and patient intake run through Barti, our scheduling and intake provider. Information you enter there is handled under their safeguards and our patient-care policies.

Contact by phone or email

If you call or email the office, we use your information to respond and to coordinate your care.

Site analytics

The site uses Cloudflare Web Analytics, which collects non-identifying usage data (pages visited, device type, page load speed) to improve the site. It sets no cookies and does not try to identify individual visitors.

Embedded content

Pages may embed maps (Google Maps) and video; those providers may set their own cookies when you interact with them.

03What we don’t do

No selling, no advertising lists.

• We do not sell personal information.
• We do not share form submissions with advertisers.
• We do not use patient information for marketing without consent.
• Email and web forms are not a safe channel for urgent medical issues — call 310-828-2010 instead.

You may contact the office to ask what information we hold from website interactions, request corrections, or ask us to delete non-record communications. California residents may have additional rights under the CCPA. For anything involving your medical record, HIPAA procedures apply — ask the front desk.
